Request for Funding 2011-2012 School Year

This form should be used to request PTO funding for
1) Friends of Kolter expenditures
2) unbudgeted needs arising throughout the year.

This form may be completed by Kolter staff, parents, or other parties. Please complete the form and attach the relevant supporting documentation. If you are unable to submit the documentation electronically, place the items in the President's Envelope in the PTO drawer in the Kolter front office.
